Memory Box Order Form:
Memory Boxes are a great way to gather and organize kid's papers from school, drawings, and other keepsake items that come home with them over the years.

 

There are 2 options to order your Memory Box: 

 

Memory Box Local Delivery: Fort Walton Beach and surrounding area - $50

Includes file box (with lid) that includes labeled hanging file folders of your color choice, 2 vinyl stickers with your child's name, and optional graphic.

 

OR

 

DIY Memory Box Kit - $35

Includes labeled file folders of your color choice, 2 vinyl stickers with name and optional graphic, shipped to you (outside of Fort Walton Beach, Florida). File box NOT included. With this option, you purchase your own file box, and we will send instructions on how to put it all together!   

 

Labeled tabs will be on the file folders and include Preschool, Pre-K, Kindergarten through 12th grade, as well as folders for Artwork and Cards/Notes.  There are also blank folders in the back that can be used for extracurriculars or a special time in your child's life.  **These labels can be customized, just add your changes to the form below.
